Duly filled Histopathology Requisition Form (Form 2) is mandatory with a copy of following documents: Doctor's referral letter with site of biopsy & clinical history; Histopathology report if paraffin block is submitted; Radiology reports: CT/ MRI/ PET Scan/ USG/ X ray.
