Preparation / Prerequisite
Submit formalin fixed paraffin embedded tissue block. Ship at room temperature. Block should have at least 10% tumor tissue OR 3 mL (2 mL min.) whole blood from 1 Lavender top (EDTA) tube. Ship refrigerated. DO NOT FREEZE. The submitted paraffin block must have atleast 10% tumor tissue. Duly filled Genomics Clinical Information Requisition Form (Form 20) is mandatory.
