Bilirubin estimation in ascitic fluid is performed to evaluate the cause of peritoneal fluid accumulation and to differentiate between benign ascites (e.g., due to portal hypertension) and pathological collections such as bile leak or perforated viscus. The test measures the concentration of bilirubin (total and sometimes direct) in ascitic fluid and compares it with serum bilirubin levels to detect bile peritonitis or bile leakage into the peritoneal cavity.
