The Free PSA test measures the amount of unbound (free) Prostate Specific Antigen circulating in the bloodstream.
PSA exists in two main forms in the blood:
1. Free PSA (unbound form) – not attached to any protein
2. Bound PSA (complexed form) – attached to blood proteins like α1-antichymotrypsin
The Free PSA level and its ratio to Total PSA help distinguish benign prostate conditions (like BPH or prostatitis) from prostate cancer.
